Membership Terms and Conditions


By signing up for membership you agree to the following terms and conditions:


Sunset Park Provisions Ministry Members willingly participate in events organized by Jane Bond, LLC DBA Sunset Park Provisions ('SPP').  I recognize the inherent risks and take complete responsibility for my involvement in membership events and membership based accessible products. 

RISK ASSESSMENT AND RELEASE OF LIABILITY:

  • I understand that SPP, its volunteers, workers, and Hosting Location are not liable for errors, omissions, acts, or failures during the event.
  • I release, waive, and forever discharge SPP from any liability, including the use of my name, likeness, image, voice, and appearance in photos and videos for marketing purposes.

CONSENT FOR MEDICAL TREATMENT:

  • I grant permission for any emergency first aid or medical treatment necessary during my participation.
  • I confirm sobriety and agree to disclose any prescription medications, herbal remedies, or pre-existing physical/mental conditions.

HEALING MODALITIES AND JOURNEY WORK:

  • I acknowledge that SPP may incorporate various healing modalities, including but not limited to plant medicine ceremonies, somatic experiencing, sound healing, breathwork, qigong, and others in the workshops and journey work.
  • I understand the nature of these modalities and willingly participate, assuming full responsibility for any experiences that may arise.

MARKETING AGREEMENT:

  • I grant SPP the absolute right to use my name, likeness, image, voice, and appearance for marketing purposes.
  • I release SPP from any liability arising from the use of such material.

GOVERNING LAW AND AGREEMENT DETAILS:

  • This agreement is governed by California laws.
  • Any modifications must be in writing and signed by both parties.

RELEASE AND WAIVER OF CLAIMS:

  • In consideration of my application, I release and waive all claims against SPP for physical, emotional, or mental injury or death resulting from negligence, breach of warranty, or strict liability.
  • I agree not to bring suit for any illness, injury, or death. I indemnify SPP from any liability arising from property damage, economic loss, personal injury, or death during the activities.

BINDING AGREEMENT:

  • This release is binding upon my heirs, successors, assignees, legatees, administrators, and personal representatives.


SUNSET PARK PROVISIONS EVENT/CEREMONY/TREATMENT WAIVER

VOLUNTARY PARTICIPATION AND ACCOUNTABILITY

I acknowledge my voluntary application to participate in the service or event organized by Jane Bond, LLC DBA Sunset Park Provisions (herein referred to as ‘SPP’). It is my choice to partake in this event, and I accept full responsibility for understanding the potential hazards involved.

ASSUMPTION OF RISK; RELEASE, WAIVER, INDEMNIFICATION, AND COVENANT NOT TO SUE

I understand that SPP, its volunteers, workers, and Hosting Location are not liable for errors, omissions, acts, or failures during the event. I release, waive, and forever discharge the Facilitators from any liability related to my participation, including the use of my name, likeness, image, voice, and appearance in photos and videos for marketing purposes.

CONSENT FOR MEDICAL TREATMENT

I consent to any emergency first aid or medical treatment required during my participation in this outing. I confirm that I am not under the influence of alcohol or illegal drugs and will notify the Facilitators of any prescription medications, herbal remedies, or pre-existing physical/mental conditions such as heart disease, diabetes, claustrophobia, asthma, bone injuries, head injuries, stroke, lung disease, etc.

AGREEMENT DETAILS

I grant SPP the absolute right to use my name, likeness, image, voice, and appearance for marketing purposes. I release the Facilitators from any liability arising from the use of such material. This agreement is governed by California laws, and any modifications must be in writing and signed by both parties.

To induce acceptance of my application, I release and waive all claims against the Facilitators for physical, emotional, or mental injury, or death, resulting from negligence, breach of warranty, or strict liability. I agree not to bring suit for any illness, injury, or death, and I indemnify the Facilitators from any liability arising from property damage, economic loss, personal injury, or death during the activities.

This release is binding upon my heirs, successors, assignees, legatees, administrators, and personal representatives.

I have read and understood the terms of this waiver before participating in SPP membership benefits and events.
